Biometric Information
For purposes of this policy, “biometric information” means any information that is collected through an identification process for individuals based on their unique behavioral or physiological characteristics, including fingerprint, scan of hand or face geometry, facial recognition or iris or retinal scans.
As a means to provide a secure and safe work environment, Wockhardt USA / Morton Grove Pharmaceuticals collects biometric information from employees and may utilize that information to identify employees, to control access to the employer’s facilities, and/or to limit access to confidential data and secure locations.
A biometric reading is information collected through an identification process for individuals based on their unique physiological characteristic. The biometric reading of the finger will be stored in a confidential computer file that is kept in the Human Resources department. The fingerprints stored are not an image file; rather they are stored as a numerical value that is generated by a proprietary algorithm. This information is collected to identify employees, to control access to the facility and secure locations. Information received is protected by the Health Insurance Portability and Accountability Act, (HIPAA) and Biometric Information Privacy Act.
Signing the signature page denotes consent from the employee. Employee biometric information shall be collected, transmitted and stored in a manner designed to protect it from disclosure. Employee biometric information shall be used solely for identification and fraud prevention purposes.
The sale, lease, or other disclosure of employee biometric information to another person or entity other than Wockhardt USA / Morton Grove Pharmaceuticals is prohibited unless:
- The employee consents, in writing, to the disclosure;
- The disclosure is required by court order, a valid warrant or a subpoena;
- The disclosure completes a financial transaction authorized by the subject; or
- Disclosure is required by state or federal law.
Wockhardt USA / Morton Grove Pharmaceuticals shall discontinue the use of an employee’s biometric information upon either of the following occurrences:
- When the initial purpose for obtaining the information has been satisfied or within three years of the employee’s last interaction with Wockhardt USA / Morton Grove Pharmaceuticals whichever occurs first; or
- The receipt of a written request to discontinue the use of such information from the employee.
In the event the employer discontinues the use of an employee’s biometric information in accordance with the provisions above, Wockhardt USA / Morton Grove Pharmaceuticals shall permanently destroy all of that employee’s biometric information within 30 days after the use of such information is discontinued.
